Admission Conditions for Patients:

1. **Health Assessment:**

- All patients are required to undergo a health assessment to determine their medical condition and treatment needs.

2. **Financial Arrangements:**

- Patients or their guardians must discuss the financial arrangements with the Responsible Director or Founder Director, as the fees vary based on the required room and the patient's disability status.

3. **Health Board Report:**

- A Health Board Report must be obtained from the Health Board Polyclinic of State Hospitals. This report is necessary for admission and should be submitted along with the other required documents.

4. **Guardian Decision (for Mental and Emotional Disabilities):**

- If the patient is mentally or emotionally disabled and requires guardianship, the guardian must provide a Guardian Decision obtained from the courthouse.

5. **Required Documents:**

- The following documents must be submitted for admission:

- A petition submitted by the disabled individual's family member or guardian to the Institution.

- A copy of the disabled individual's ID card.

- Four recent photographs of the disabled individual.

It's essential to ensure that all necessary documents and requirements are met for the admission process to proceed smoothly.

Admission Conditions for Patients Receiving Government Funding

Application Procedure for Individuals Whose Expenses are Covered by the State (AÇSHB):

1. **Health Assessment:**

- It is mandatory for the patient to be fully dependent.

- A Health Board Report must be obtained from the Health Board Polyclinic of State Hospitals.

2. **Income Assessment:**

- The per capita income resulting from dividing the total income of the household where the disabled individual resides by the number of individuals in the household must be less than 3/2 of the minimum wage. For example, if there are 3 people living in the household including the disabled individual, and if the total income of the household is 4000 TL with 2 individuals having official salaries or incomes, the per capita income would be 4000/3 = 1333 TL. Therefore, if the per capita income is less than 1402 TL, and the disabled individual meets other requirements, their expenses can be covered by the state.

3. **Application Process:**

- Families meeting these criteria must apply to the Directorate of Family, Labor, and Social Services in their province/district with the documents listed below.

Documents Required for Admission with Government Funding:

1. **Health Board Report** (Full dependency is mandatory).

2. **Guardian Decision** (If newly applying for guardianship or if it has been recently approved, a "Transfer Form" must be obtained from the courthouse).

3. **Petition to the Directorate of Family, Labor, and Social Services** in the province/district where the disabled individual resides. (The petition must specify the name of our institution. For example, "I want to place my disabled family member in the Private Gardenya Care Center in Isparta province").

4. These documents will be directed by the guardian to the Social Service Specialists in the district governorship for residential care services.
